This form must be reviewed, understood and signed before participation in any glass blowing or glass working activities. A copy will be provided upon request.
I understand that glass blowing and working with glass are inherently dangerous activities. For example, glass blowing involves working with molten glass at 2050 degrees. The tools on which glass is shaped may weigh as much as 20 pounds and may be as hot as 1500 degrees. Glass, while being shaped and blown, may be 1500+ degrees. All glass working tools, will absorb heat during the process of working with glass. These facts create risks and dangers of burns which can be serious. Broken glass and glass shards commonly exist in the studio and can cause serious cuts.
By signing this release form, and participating in this glass blowing class, I am voluntarily and knowingly accepting all risks inherent in this activity. I will follow all instructions. I will act carefully and thoughtfully. I will ask questions if I am unsure what to do. I will wear closed shoes and utilize any needed safety equipment, including heat shields. If the equipment seems too heavy or if the glass seems too hot, I will tell the staff or instructor immediately. I have been notified of the safety procedures and location of first aid items.
If I have any medical, mental or physical condition that may possibly increase the inherent risk, or make it more difficult to engage in glass blowing activities, I will notify Glen Echo Glassworks or my instructor before engaging in the activity. A form to indicate any such medical, mental or physical condition is attached and must be completed and discussed with Glen Echo Staff or instructor prior to beginning any class. Except for any issues disclosed on the medical information form, I certify I have no medical issues that would affect my class participation.
By signing this release form, I expressly and fully release Paul Swartwood d/b/a Glen Echo Glassworks, his agents and instructors, and Glen Echo Park Partnership for Arts and Culture from any and all liability for any injury, damage or loss to any person or property which may arise directly or indirectly from these glass blowing activities. This waiver specifically includes liability for negligence by Glen Echo Glassworks, its instructors or staff, Glen Echo Park Partnership for Arts and Culture, other participants in the class, observers and myself. This release will be effective for all sessions of glass instruction, or for the duration of individual lessons.
